Eliza Acdan
Hello, my name is Eliza Acdan. I am a recent graduate and I just received my diploma in the mail. Upon opening it, I was surprised to find that my middle name was missing from the document. I had called and visited the registrar office multiple times before this to make sure my middle name - Layugan - would be included on my diploma when I received it. When I visited the office in person, they assured me that it would be included as requested.
Please get back to me about how to resolve this issue.
Thanks,
Eliza Claire Layugan Acdan
